Personalized Care Plan
Once we fully understand your loved one's needs, we’ll collaboratively craft a care plan that’s uniquely theirs—whether it’s assistance with personal care, therapy, companionship, or medication reminders. After your approval, we guide you through a simple service agreement and introduce your dedicated caregiver, ensuring a seamless transition into care.

Your caregiver will start with an introductory visit, reviewing the care plan together to ensure alignment. What is a veteran care program & how do i fit in?
At Patriots Pathway, a Veteran Care program helps eligible veterans get the support they need while staying in the place they love most — home. We work hand-in-hand with the program to make the process simple. From figuring out if you qualify to handling the paperwork and matching you with the right caregiver, we’re with you every step of the way.
How do i know if i qualify?
If you’re enrolled in VA healthcare and need help with everyday activities like bathing, getting dressed, cooking, or moving around safely, you may be eligible. Some programs even extend to spouses or surviving spouses. Don’t worry — you don’t have to figure it out alone. We’ll walk you through the eligibility steps and help you gather what’s needed.
what kind of help can i get at home?
